About The Position
The Certified Community Health Worker is responsible for serving as the initial and main point of contact between the company and the member. Supports member access to care through face-to-face visits to complete health screening and needs assessments. Certified Community Health Worker refers members to community resources, processes reports, and distributes collateral materials.
