Weber Human Services (WHS), located in Ogden, Utah serves a growing urban population residing in Weber and Morgan Counties. We are an organization dedicated to helping those in need rebuild their lives and reach their full potential. WHS was organized by a cooperative agreement between the Weber and Morgan county governments to serve as the local authority for the provision of their substance abuse, mental health, and senior service programs. Weber Human Services is the only organization in the State of Utah providing these services under a single administration. Through dozens of human services programs, WHS helps thousands of its neighbors by supporting and empowering our most vulnerable groups, including at-risk youth, the elderly, children, youth and adults suffering from mental illnesses, and those recovering from addictions. Weber Human Services creates hope and opportunities that enable all clients and staff to: do the work necessary to become their best self and find joy through people, place, and purpose. This position is located in our Day Treatment program. In this Case Manager position your duties will include; picking up clients from group homes and annex each morning and take them back after lunch. Participate as part of a team for the morning group from 9-noon. Educate clients on Daily Living Activities on an individual and group basis. Link clients to different resources with Weber Human Services. Link clients to different resources outside of Weber Human Services including Medicaid, social security, general assistance, food stamps, housing, food resources, etc. Take clients to the main building for lab work and injections. Connect clients to AA/NA and other support groups in the community. Assist clients in completing step work towards their recovery. If a PSS, use personal life experience to assist clients in their recovery of mental health and substance use. Teach skills to clients in their apartments, homes and/or group homes. As a Certified Case Manager for our Adult Mental Health Team, you will work with adults who need community resources. You will need to have experience working with mental illness and be willing to work in the community, schools, hospitals, residences, shelters, etc. You must have strong collaboration and communication skills and be willing to work a flexible schedule if needed. We will provide training to become a Certified Case Manager. Spanish speaking is a plus, and a stipend will be available upon passing the Spanish competency test.
