Its focus is advocating and implementing integrated care model approach to underserved population undergoing behavioral health related issues. POSITION SUMMARY:  The Certified Addictions Counselor (CAC) provides direct substance use disorder (SUD) treatment services through group and individual counseling, psychoeducation, and care planning. Utilizing a person-centered, trauma-informed approach, the counselor supports clients in achieving sobriety and wellness goals. The role includes assessment, treatment planning, documentation, and collaboration with interdisciplinary teams.

Requirements

  • Bachelor’s or Master’s in behavioral healthfield.
  • DC licensure: CACI or CAC II
  • 2+ years SUD counseling experience preferred.
  • Familiarity with ASAM criteria and evidence-based modalities.
  • Proficiency in MS Office applications (Word, Outlook, Excel, Adobe) and various business software.
  • Legally authorized to work in the U.S.A.

Responsibilities

  • Maintains a caseload as assigned and in accordance with COMAR and CARF regulations.
  • Ensures incoming clients have a complete psychosocial history and comprehensive assessment and interpretative summary within 5 days of admission.
  • Completes, with team participation, a Preliminary Treatment Plan within 3 days of admission.
  • Prepares, with team and client participation, a Comprehensive Treatment Plan for all patients within30 daysof admission.
  • Completes a Treatment Plan Update every30 days following the completion of the Comprehensive Treatment Plan.
  • Prepares an aftercare plan for all patients completing the program or being discharged for other reasons.
  • Provides follow-up care and contact with each discharged patient.
  • Provides individual addiction counseling according to the Comprehensive Treatment Plan.
  • Provides coordination of care o fHealth Home services according to the Treatment Plan.
