About The Position

This is a physician-led and mission-driven primary care organization focused on transforming care for the neediest senior population. The mission is to honor seniors with affordable VIP care that delivers better health. The organization seeks primary care providers and clinical leaders to fulfill this mission. The role involves a small patient panel of 60-90 seniors with 3-5 chronic diseases, operating within a value-based care model. The clinic offers in-house consulting specialists (Cardiology, Podiatry, etc.), holistic health services (Acupuncture, Tai Chi, Yoga, Nutrition), and provides door-to-door transportation for patients.

Requirements

• Board Certified in Family Medicine, Internal Medicine, or Geriatric Medicine
• Clear/Active Medical License in state of employment
• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty
• At least 1 year of clinical experience in a geriatric or family practice setting
• Strong leadership skills
• Strong communication skills
• Strong analytical skills
• Proficiency in Microsoft Office
• Ability to travel locally and occasionally regionally
