Center Manager - Full Time Under the general supervision of the Center Administrator, the Center Manager/IDT Team Leader will have authority and responsibility for the management of the Interdisciplinary Team activities and staff. The IDT Team Leader will fulfill the regulatory role of the Center Manager. Center Manager will ensure that decisions pertaining to participant services are driven by participant needs and the IDT decision making process. All employees are expected to be knowledgeable and compliant with Carolina SeniorCare and Elderhaus PACE values of compassion, dignity, excellence, integrity and teamwork.

Requirements

  • Associates Degree or higher by an accrediting agency recognized by the US Dept. of Education in HealthCare related field.
  • Professional licensing as required by position.
  • Cardio Pulmonary Resuscitation (CPR) certification required.
  • Prefer 2-5 years’ experience in health care related field with at least 1 year supervisory experience.
  • Minimum of 1 years’ experience working with the frail elderly population.

Responsibilities

  • Embraces the Carolina SeniorCare Elderhaus PACE goals to enhance function, mobility, independence, quality of life, and quality of end of life of participants and holds accountable direct reports to support the organizations goals.
  • Coordinates all IDT Meetings including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences and all related follow up.
  • Directs and oversees the IDT members. Hires, trains, evaluates and disciplines IDT Members in coordination with the Center Administrator. Coordinate staff development and training and assists with other human resource issues in conjunction with Human Resources Department.
  • Responsible for the implementation, coordination, and development of quality improvement processes as it relates to the Interdisciplinary team.
  • The Team Lead/Center Manager is responsible for the development, implementation, and execution of participant care.
  • Facilitates activities related to new participants in coordination with the IDT.
  • Emphasized teamwork and collaboration in all clinic and Interdisciplinary interactions.
  • Performs routine care plan audits and all related follow up with disciplines’ interventions and evaluations in participants’ care plans.
  • Ensures that team members understand the importance of their and others’ roles on the team and remain open to the cooperative dynamic of the team.
  • Ensures IDT team members practice effective time management skills.
  • Ensures the provision of quality service within expected standards of care.
  • Coordinates orientation and develops in-service programs for staff to meet regulatory requirements and support performance improvement.
  • Meets regularly with direct reports to review goals, trends, and performance measure to facilitate change as needed.
  • Functions as a member of leadership team with the Carolina SeniorCare Elderhaus PACE organization and promotes the mission and vision of leadership in a positive and productive manner.
  • Complete paperwork for new hires, check references, make offer, give appropriate paperwork to Human Resources
  • Process payroll, (check timecards for errors, enter missed punches, sick time and PAL time) submit to Human Resources for errors
  • Prepare and develop training materials for department specific orientation for new hires
  • Record employee accomplishments
  • Complete new hire and annual competency documents for employees and return to Human Resources
  • Hiring and staffing for department, coordinate team interviews, and interview candidates
  • Coach and develop existing employees to ensure satisfaction and growth
  • Deal with performance problems and terminations and conduct timely performance evaluations
  • Create schedules for employees in department
  • Approve/disapprove requested time off for employees in department, subject to available PAL balances
  • Complete absence sheets timely and return to Human Resources
  • Complete disciplinary paperwork timely and return to Human Resources
  • Performs other duties as assigned.
