About The Position

The CDOC Community Behavioral Health Case Manager is a term-limited position focused on providing case management services to inmates transitioning from a correctional facility to the community and parole supervision. The role involves supporting parolees with mental health and substance use disorder treatment needs, coordinating with various community resources, and ensuring compliance with fiscal policies and procedures. This position is located in Denver, Colorado, and is open only to Colorado state residents.

Requirements

  • Bachelor's degree in counseling, social work, psychology, or marriage and family therapy from an accredited institution.
  • Related experience in mental health services may substitute for the degree requirement on a year-for-year basis.
  • Ability to provide direct mental health support services to clients.
  • Excellent written and verbal communication skills.
  • Excellent interpersonal skills for collaboration across facility programs and with external agencies.
  • Ability to work independently under supervision.
  • Ability to provide comprehensive clinical documentation.
  • Ability to work with a diverse population of clients.
  • Ability to provide mental health support services as part of a multi-disciplinary team.
  • Ability to read and comprehend the English language.
  • Ability to create accessible documents and technical content.

Responsibilities

  • Provides case management services to inmates transitioning from a CDOC facility to the community and parole supervision.
  • Supports parolees with moderate to high mental health and substance use disorder treatment needs.
  • Identifies inmate behavioral health, MAT, and substance abuse treatment case management needs using assessment tools.
  • Works directly with Community Behavioral Health Clinicians and parole staff to identify parolees in need of case management services.
  • Schedules and facilitates behavioral health appointments in the community.
  • Tracks inmate participation in behavioral health treatment services.
  • Submits timely and accurate reports, plans, and other documents.
  • Coordinates with parole community re-entry services and community resources to address housing, employment, and transportation needs.
  • Decides appropriate peer recovery support referrals.
  • Participates in community behavioral health planning, meetings, and training.
