About The Position

CDDO Eligibility Specialist Navigating Services, Empowering Lives. Position Summary This position is responsible for determining eligibility for Intellectual/Developmental Disability (I/DD) services, conducting functional assessments, and providing information about service options and community resources within the Community Developmental Disability Organization (CDDO) area.

Requirements

  • Bachelor's degree in human services or related field with 2 - 3 years of experience
  • Experience in the I/DD service field
  • Valid Driver’s License and driving record acceptable to insurance carrier is required. Must be able to pass all background checks
  • Proficient in determining eligibility for I/DD services and conducting functional assessments
  • Knowledge of the I/DD system, affiliated service providers, and Home and Community-Based Services (HCBS)
  • Ability to communicate effectively with individuals, guardians, and service providers
  • Organizational skills to manage documentation, data entry, and compliance with contract requirements
  • Understanding of consumer rights and provider requirements within the I/DD system
  • Familiarity with Microsoft Office and online platforms such as Teams and Zoom
  • Ability to guide individuals through assessments and communicate results clearly
  • Effective networking and collaboration skills to ensure appropriate communication regarding CDDO services

Responsibilities

  • Determine program eligibility for individuals seeking I/DD services in designated counties.
  • Complete eligibility re-determinations for other CDDO areas upon request.
  • Serve as the CDDO intake/application contact in the absence of the CDDO Eligibility & Referral Manager.
  • Assist with maintaining and reviewing data for accuracy and reliability in the CDDO database.
  • Provide information regarding community service options and resources for individuals with disabilities and make referrals to other social service organizations as needed.
  • Complete initial, annual, and special re-evaluation functional assessments for assigned individuals and other CDDO areas as requested.
  • Ensure timely and accurate data entry of functional assessments into the Kansas Department for Aging and Disability Services (KDADS) database.
  • Disseminate functional assessment results to individuals, guardians, and community service providers.
  • Upload documentation into the KDADS database & the CDDO's electronic filing system for compliance reviews.
  • Maintain eligibility to conduct program eligibility determinations and functional assessments through required training.
  • Collaborate with interdepartmental staff and participate in CDDO workgroups as requested.
  • Maintain positive, professional relationships with clients, co-workers, and community representatives.
  • Handle sensitive information with confidentiality and discretion in accordance with HIPAA regulations.
  • Maintain a professional and positive image of the organization, and demonstrate commitment to the organization's mission and values.
  • Demonstrate zero tolerance for abusive, neglectful, and/or exploitative acts towards individuals served. Report any suspected incidents in accordance with Cottonwood CDDO Policies and Procedures.
  • Protect and promote the rights, dignity, opportunities for choice, health and safety of persons served. Demonstrate caring relationships with each individual you work with using lavish praise and positive reinforcement.
