About The Position
Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency, and we are currently seeking a Care Coordinator for our South Chicago, IL location. This full-time position is ideal for a dynamic individual who is passionate about providing quality care to the elderly. The Care Coordinator will be responsible for conducting determination of eligibility and need assessments, performing facility placement pre-screening assessments, and authorizing Community Coordination Program (CCP) Services for all eligible participants in the Illinois Department on Aging's CCP program. Key functions of the role include the administration of the appropriate intake forms, which encompass a comprehensive needs assessment. The Care Coordinator will also be tasked with developing a case plan tailored to the individual needs of clients, making appropriate referrals, and responding to applicant and client requests. Additionally, the role involves the authorization of services and maintaining detailed case records, which include documentation of follow-up, reassessment, and termination of services. This position requires a strong commitment to ensuring that all clients receive the necessary support and services they need to thrive in their home and community environments. The ideal candidate will possess a Bachelor of Arts or Bachelor of Science degree in social science, social work, or a related field, along with at least one year of experience in case management and home care. Bilingual proficiency in Spanish and English is required, as is a valid driver's license and a personal vehicle with insurance. The Care Coordinator will work a standard 8-hour shift from Monday to Friday, and the role requires 100% travel to meet with clients in their homes or other locations.
