About The Position

We are seeking a CCM Supervisor to oversee the day-to-day operations of our Chronic Care Management team. This leader will monitor performance metrics, ensure scheduling efficiency, support onboarding and training, and help drive alignment with business unit goals. The ideal candidate will be a licensed Medical Assistant (MA), Licensed Vocational Nurse (LVN), or Licensed Practical Nurse (LPN) with leadership experience and a passion for patient-centered care.

Requirements

Active MA, LPN, or LVN license
Strong communication, time management, and organizational skills
Demonstrated empathy and ability to connect with patients
HIPAA-compliant, quiet home workspace
Personal computer and reliable internet connection
Moderate to advanced computer skills
Familiarity with EHR systems, scheduling tools, and reporting dashboards

Nice To Haves

Prior leadership or supervisory experience in a clinical or virtual care setting (preferred)

Responsibilities

Monitor and maintain real-time performance reporting (call minutes, volume, etc.) for CCM agents
Ensure team alignment with performance and productivity targets
Oversee onboarding and training of new hires
Set up Calendly links for newly hired agents and manage their schedules
Post weekly CCM team schedules and adjust as needed
Ensure skill-based voicemails and patient text messages are followed up on timely
Review and address callback forms and rescheduled patient calls
Step in to ensure scheduled patient calls are completed if an agent is unavailable
Cultivate a positive, collaborative, and accountable work environment

Perform other related duties and projects as needed
