About The Position

The Licensed Vocational Nurse (LVN) position at La Clínica de La Raza is a temporary part-time role focused on delivering patient-centered care in a community health setting. The LVN will engage in health coaching, manage patient registries, and collaborate with healthcare team members to improve patient outcomes. This role requires a blend of clinical skills, effective communication, and the ability to work in a fast-paced environment while providing compassionate care to diverse patient populations.

Requirements

• Valid license to practice as a Vocational Nurse in the State of California.
• One to two years' experience in an outpatient facility working with adults and children.
• Current BLS (Basic Life Support) Certificate, with a 30-day grace period to obtain it from the date of hire.
• Experience working with patients in health promotion, with knowledge of medical terminology and chronic health conditions.
• Bilingual in English and Spanish.

Nice To Haves

• Knowledge of chronic care model and fundamentals of health education.
• Proficient in managing patient databases and software programs such as Excel and practice management systems.

Responsibilities

• Participate in health coaching activities such as medication adherence and counseling patients on healthy behaviors.
• Update and maintain a registry of patients using a robust tracking system.
• Assist in training and support Medical Assistants in panel management.
• Systematically review registries for target populations and conduct predetermined interventions.
• Contribute to quality improvement efforts by providing feedback and progress reports to Site Improvement Teams.
• Collaborate with other healthcare team members to support individual or group-based interventions of care.
• Enter data and manage its integrity for patient registries, conducting data cleanup and running periodic reports.
• Carry out provider/health care team orders consistent with level of training, including scheduling appointments and coordinating referrals.
• Attend section meetings and other meetings that support job responsibilities.
