SUMMARY OF JOB In this role, the CCC/IWA Manager provides hands on operational support and communication to facilitate daily operations of the Community Care Collaborative (CCC) and Innovative Wellness Alliance (IWA). The CCC/IWA Manager proactively manages the operational aspects of various value based contracts such as the CMS Shared Savings program, Aetna, Cigna, DevotedHealth, Humana, Tandigm and TVOC, as well as conducting related administrative requirements. The ACO/IWA Manager is the primary liaison to CMS, third party payers and Population Health vendor for operational matters and is responsible for the planning, coordinating, communicating and executing information technology and other CCC/IWA projects and initiatives and support CCC/IWA governance communications at the direction of the Executive Director and/or the AVP of Finance for HCD/Population Health. The CCC/IWA Manager will oversee the population health team consisting of Population Health Supervisor and Guides, as well as a Certified Professional Coder to provide primary support to the CCC/IWA participants/providers, as well as the CCC/IWA leadership and related committees. The CCC/IWA Manager will support administrative and clinical leadership to gather feedback on projects and assignments. The Manager will provide oversee the various CCC/IWA committees and workgroup sessions, help set the agenda, communicate timing and logistics and manage the meeting and document meeting notes, outcomes and next steps.

Requirements

  • 5 years’ experience in ACO quality measure management and reporting, population health, hospital and/or Healthcare Administration, Information Technology or Health Care Finance.
  • Supervisory experience preferred.
  • Bachelor's degree preferred, not required, or equivalent work experience.
  • Preferred experience with Accountable Care Organization program model, primary care practices value based risk contracting. Regulatory knowledge a plus.
  • Experience with workflow and EMR documentation development.
  • Demonstrated experience with analysis and quality improvement activities.
  • Advanced/Proficient Microsoft Office Suite (Word, Excel, Access, Visio) user as well as Data Analytic tools such as Power BI.
  • Relevant competencies include developing and maintaining customer relationships, analytical problem solving, administrative operations and project oversight, effective written and verbal communication and strategic insight.
  • Must be able to work in a team environment and exhibit verbal and written communication skills, flexibility and enthusiasm in learning new information and developing new skills quickly.
  • Lean Six Sigma or PM certification a plus.
