About The Position

The Integrated Care Coordinator serves as the primary liaison ensuring that each individual receiving CCBHC services has access to coordinated, comprehensive, person-centered care. This role integrates behavioral health, primary care, and social support systems to improve outcomes for individuals with behavioral and physical health needs. Requirements

  • High School Diploma or GED
  • Strong interpersonal and organizational skills.
  • Proficiency with EHR systems and documentation standards
  • Knowledge of community health resources and behavioral health systems.

Nice To Haves

  • Prefer 2+ years of experience in behavioral health or case management, ideally within an integrated or CCBHC environment, but willing to provide training to new graduates.

Responsibilities

  • Facilitate linkage between behavioral health, medical and social service providers in alignment with agency policies on service coordination.
  • Ensure timely referrals, follow-up appointments and feedback loops among treatment team members and external agencies.
  • Maintain comprehensive and up to date treatment plans and progress documentation.
  • Serves as a primary advocate ensuring the person’s needs, preferences, and goals are all care decisions.
  • Engage families and support systems as permitted and ensure informed participation in all care planning.
  • Coordinate across all disciplines (therapists, nursing, psychiatry, peer support, case management) to ensure service delivery.
  • Support warm handoffs between CCBHC services such as crisis, outpatient, MAT and community programs.
  • Establish and maintain collaborative relationships with local healthcare providers, community agencies and referral sources.
  • Assist in identifying community resources and addressing service gaps.
  • Uphold CARF and CCBHC standards, ensuring all linking and coordination procedures are documented and compliant with organizational policies.
  • Participate in CQI initiatives and audits related to coordination and service linkage.
  • Ensures compliance with Federal, State, and local regulations and RiverValley’s mission policies, procedures and standards.
  • Completes all mandatory compliance, HIPAA and state and federal regulatory training as required.
  • Maintains appropriate degree of confidentiality.
