About The Position

Center for Human Development, CHD, is currently seeking a Caseworker to join our Basic Center Emergency Shelter Program in West Springfield. The CHD Basic Center Program (BCP) provides low-barrier services and supports to runaway and homeless youth (RHY) ages 14–17. Services include crisis intervention, prevention and referral, counseling, leadership development, and temporary shelter. Through the BCP shelter program, youth are placed in licensed CHD host homes for up to 21 days. During this time, staff work to reunify youth with their families whenever possible or identify appropriate alternative placements that support long-term stability. Your role as a Caseworker: Develop and implement, in partnership with each youth, an Individualized Service Plan addressing housing, education/employment, health and well-being, and permanent connections, with a focus on stabilization and long-term success. Complete intakes, assessments, and ongoing evaluations of youth participating in the program. Conduct regular home visits and well-being checks; develop and monitor safety plans for youth in host homes. Support youth in developing transition plans aimed at securing safe, stable, and preferably long-term housing upon discharge. Serve as liaison with host home providers to ensure a safe, supportive environment and adherence to program standards of care. Requirements

Bachelor’s degree in a related field preferred, or a minimum of two years of relevant experience.
Ability to pass CORI background check and driving record screening.
Ability to obtain CPR and First Aid certification.
Valid driver’s license, reliable transportation, and active vehicle insurance.
Demonstrated ability to work respectfully with diverse populations, including LGBTQ+ youth and other vulnerable groups.
Strong verbal, written, and organizational skills; ability to work both independently and as part of a team.
