About The Position

This is a grant-funded position. Ongoing employment is contingent on continued grant funding. Performs social services work by serving as a liaison between individuals and their families in obtaining assistance and rehabilitative treatment. The Collaborative Care Case Manager provides outpatient mental health–related care coordination services for individuals with intellectual and developmental disabilities (IDD) who have co-occurring mental health conditions, substance use disorders, or behavior support needs. This position delivers collaborative care case management using a cross-systems, biopsychosocial approach and works in coordination with medical, behavioral health, and IDD service providers to improve access to care, reduce crises, and support successful community living. The Outpatient Biopsychosocial Intervention (OBI) Collaborative Care Case Manager may also provide skills training services consistent with state-funded mental health psychosocial rehabilitation models.

Requirements

  • Bachelor’s degree or higher from an accredited college or university in a social, behavioral, or human services field (e.g., psychology, social work, nursing, counseling, rehabilitation, sociology, human development, or related field).
  • Apply trauma-informed care principles, including cultural sensitivity, in all aspects of service delivery.
  • Communicate effectively, both verbally and in writing.
  • Exercise sound judgment and problem-solving skills in a variety of situations.
  • Demonstrate behavior consistent with the Center’s values and Policies and Procedures.
  • Work collaboratively and maintain effective relationships with supervisors, individuals served, coworkers, and community stakeholders.
  • Proficient in computer applications such as Microsoft Word and Excel.
  • Must have and maintain a valid drivers’ license with an acceptable driving record.

Nice To Haves

  • Experience working with individuals with intellectual and developmental disabilities and co-occurring mental health or behavioral support needs preferred.

Responsibilities

  • Provide collaborative care case management services to individuals enrolled in the OBI program in accordance with HHSC contract requirements.
  • Coordinate care across physical health, behavioral health, and IDD systems to support accurate diagnosis, effective treatment planning, and continuity of care.
  • Conduct service coordination activities that reduce barriers to accessing mental health, medical, behavioral, and community-based supports.
  • Make appropriate referrals to specialized mental and physical health services, behavior supports, peer supports, and community resources.
  • Monitor and support medication management by assisting with scheduling appointments, coordinating specialty care referrals, and tracking follow-up.
  • Support individuals during transitions of care, including hospital discharges, crisis episodes, or changes in service providers.
  • Participate in consultation and coordination with interdisciplinary professionals, such as Nursing, Psychiatrists, Psychologists, Social Workers, Licensed Professional Counselors, Board Certified Behavior Analysts, and specialized medical and therapy providers
  • Communicate regularly with individuals, families, guardians, and support systems to ensure coordinated and person-centered service delivery.
  • Provide skills training to individuals, parents, and/or support systems at least monthly, as identified through the Biopsychosocial Assessment.
  • Deliver mental wellness supports and foundational skills training, including coping strategies, communication supports, and behavior support techniques.
  • Support parents and caregivers in building structure, consistency, and environments that promote positive outcomes for individuals with IDD.
  • May assist Lead OBI in conducting education and training opportunities for behavioral/mental health professionals, medical professionals, law enforcement, and other entities engaged with individuals with IDD.
  • Ensure accurate and timely documentation of all services provided.
  • Maintain required documentation for collaborative care case management services
  • Ensure services are provided in accordance with HHSC contract requirements, LIDDA policies, and applicable administrative rules.
  • Participate in team meetings, case reviews, trainings, and quality improvement activities.
  • Identify service gaps, risks, or emerging needs and communicate them to the OBI Lead or supervisor.
  • Maintain compliance with required training, professional development, and cross-training.
  • Demonstrate high ethical standards, professionalism, and personal integrity.
  • Maintain regular attendance and dependability in accordance with the established work schedule.
  • Perform related duties as assigned.
  • Frequent travel within the catchment area is required.
