CASE WORKER - JACKSON, MICHIGAN AREA D o you have a Bachelor’s degree in SOCIAL WORK or related human service field? We have a full time CASE WORKER role, assisting youth who have experienced time in foster care to establish educational goals, determine living arrangements as well as financial needs, empowering them to make positive decisions regarding their education and future. Requirements

  • Bachelor's degree in social work or other related field.
  • Unrestricted valid driver's license with good driving record.
  • Reliable personal transportation suitable for safely transporting persons served; proof of current automobile insurance.

  • Minimum one year experience in child welfare program preferred.

Responsibilities

  • Provides case management services and coordinates vouchers for education and training for young adults who are aging out, or have aged out of foster care in accordance with all applicable contractual and regulatory requirements and accreditation standards.
  • Provide Education and Training Voucher Applications to each youth interested, as required.
  • Assist youths to establish educational goals, determine living arrangements, and determine financial need that will empower youths to make informed and healthier decisions regarding their education and future.
  • Monitor and assess each youth's educational, housing, employment, and mental health needs each semester with inputs from the youth.
  • Provides community outreach support by making face-to-face contacts, telephone calls and presentations to assist identified youth.
  • Assess, identify, and coordinate use of resources such as Court Appointed Special Advocates (CASA) workers, direct services workers, educational resources, and state and private organization officials.
  • Work collaboratively with referral sources, agencies, educational institutions, and individuals who have important roles in the client's life situation.
  • Collaborate and work with high school guidance counselors to help educate and inform youth in various schools across Michigan in the ETV program and its eligibility requirements.
  • Update student profile on computer database biannually.
