About The Position

Job Summary Provides routine social services as a member of a multi-disciplinary team, including obtaining social histories, providing counseling and guidance, referrals to agencies that respond to the psycho-social needs of patients/clients and families. As appropriate identifies and refers, or seeks supervision on complex cases. Reports to Director of department or program, or designee.

Requirements

• Bachelor Degree in Human Services
• Ability to obtain Public Service License required
• Demonstrated competence including knowledge and behaviors to meet age-specific needs of the client population
• Experience working in related human service setting with similar age group
