About The Position

The Case Worker, also referred to as Patient Navigator, plays a crucial role in a multidisciplinary team at Landmark Recovery, focusing on assessing and coordinating care for patients with Substance Abuse Disorder. This position is essential for ensuring that patients receive comprehensive support during their treatment journey, both inpatient and outpatient, and helps facilitate their transition to sobriety by addressing various biopsychosocial stressors.

Requirements

  • Bachelor's Degree or higher in a behavioral science strongly preferred.
  • At least 2 years previous experience in case management required.
  • Commitment to a drug-free lifestyle and responsible alcohol use.
  • Personal passion or interest in substance abuse issues strongly preferred.
  • Strong organizational skills and ability to multitask seamlessly.
  • Ability to remain calm under pressure and demonstrate strong de-escalation skills.

Nice To Haves

  • Experience in behavioral health for at least 2 years.

Responsibilities

  • Assessing patient out of facility biopsychosocial stressors and developing a strategy to meet these needs during and after treatment episode.
  • Facilitating multiple care aspects including case coordination, information sharing, and aftercare planning.
  • Maintaining good working relationships with support networks, government resources, community resources, employers, and referral sources.
  • Completing all necessary documentation related to FMLA and Short-Term Disability.
  • Maintaining patient records by reviewing and completing case notes.
  • Preparing patients' discharge by reviewing and coordinating community services necessary to maintain sobriety outside of structured setting.
  • Participating in weekly multidisciplinary treatment team meetings as part of ongoing assessment of patient needs and discharge planning.
  • Daily involvement in census management including coordinating discharge schedules and communicating with treatment and administrative teams.
