Our desire is to see individuals reach their full potential, and create lasting change that will stand the test of time. The Case Worker II reports to the Program Director. The Case Worker II will facilitate individual programing services and activities in accordance with case management plans.

Requirements

  • Possess either a bachelor’s degree in social sciences or a related field; or a high school diploma or equivalent and a minimum of two (2) years' experience working with a criminal justice or related population
  • Ability to complete a minimum of 40 hours of in-service training hours per year. This is a contractual requirement that could be subject to change.
  • Legibly, accurately and appropriately document communications, behaviors and facility events as they occur.
  • Intermediate computer skills in Office 365, Teams, Zoom and other communication/meeting platforms.
  • Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  • Ability to pass a pre-employment criminal background check.
  • Ability to pass a pre-employment drug screen, and TB test.

Nice To Haves

  • Ability to understand and use ARMS (Automated Reentry Management System) preferred.
  • Current certified training in emergency first aid, preferred.

Responsibilities

  • Support residents in achieving their case management goals by delivering comprehensive program services, including group facilitation, structured activities, individual sessions, and other direct support services in alignment with individualized case plans.
  • Conduct resident orientations and complete thorough needs assessments to inform service planning.
  • Develop, review, and update resident case management plans on a regular basis to ensure progress and goal alignment.
  • Maintain accurate, timely, and compliant documentation, including case notes and case management plans, in accordance with the agency’s Scope of Work and CDCR/Probation requirements.
  • Monitor and document resident progress toward established goals, maintaining consistent communication with referral sources regarding achievements, barriers, and overall status.
  • Plan, coordinate, and implement structured group activities that support program objectives and resident engagement
  • Will think and act quickly and efficiently in emergencies.
  • Will drive on Agency business.
  • Physical presence at the program is required.
