Position will provide youth-centered case management/referral services for high school age youth as part of a school-based health center (SBHC) located at Jennings High School. The position will coordinate with other behavioral health/health staff members of the SBHC multidisciplinary team. The position will also coordinate closely with school faculty and leadership to implement services within the school setting. This team will create care plans to include identification of social, mental health, health prevention and educational/vocational needs. This position will be dedicated to and experienced in working with diverse youth/families of all races and sexual orientations and identities. The position will utilize assessments and screening to identify areas of concern for youth and provide community resource referral and coordination, crisis intervention and supportive counseling to assist the client/family, and coordinate access to needed resources. Will abide by the same strict standard of patient confidentiality that applies to all WashU staff.
