About The Position

The position of Case Worker I is intended to provide client-centered HIV case management services and thorough assessments for individuals in coordination with the multidisciplinary healthcare team. The position will create and implement multidisciplinary care plans to include identification of missed and kept appointments, monitor cancelled appointments, assess clients’ barriers to care, refer to needed services, and coordinate access to resources in accordance with all applicable local, state, and federal guidelines and procedures. The Case Worker I will abide by the same strict standard of patient confidentiality that applies to all WashU staff. The Case Worker I will work under the under the direction of senior staff.

Requirements

  • Bachelor's degree
  • Basic Life Support - American Heart Association, Basic Life Support - American Red Cross
  • No specific work experience is required for this position.
  • Equivalent of a bachelor’s degree in a health-related field, such as social work, counseling, sociology, or psychology.
  • Applicants with bachelor’s degrees in other fields may be considered with sufficient case management experience.
  • Basic Life Support certification must be obtained within one month of hire date
  • Basic Life Support certification (Online BLS certifications, those without a skills assessment component, are not sufficient to meet the BLS requirements).

Responsibilities

  • Conduct a comprehensive culturally sensitive assessment with the client, and family/support network as appropriate.
  • Synthesize program goals and assessed needs to create an individualized plan of care for each client.
  • Arrange and coordinate service referrals for the client/family as needed; provide ongoing monitoring of the activities implemented to evaluate progress on the plan of care.
  • Refer clients/families to public benefits programs as eligible.
  • Independently manage caseload by providing regular contact with enrolled clients and documenting all contacts and service coordination activities in client records as required by program guidelines.
  • Educate clients/families about the basic medical and psychosocial aspects of HIV/AIDS in order to optimize adherence to care.
  • Adhere to all applicable state laws and the department’s rules and regulations regarding HIV/AIDS programs.
  • Maintain current knowledge of medical and psychosocial developments concerning HIV/AIDS and related conditions, and routinely interact and network with ASOs, providers, clinicians, and others involved in HIV/AIDS service delivery.
  • Collaborate with case managers from other regions as needed when a patient resides outside of program service boundaries.
  • Perform indirect, non-clinical tasks related to the job, including compilation of data, participation in required training, and attendance at staff meetings and community meetings as assigned.
  • Maintain database and work with data team.
  • Function as part of a multidisciplinary HIV Specialty Health Care Team, and meet as scheduled in case conferences to ensure coordinated delivery of care to patient and family.
  • Educate clients/families on HIV/AIDS, medication adherence and safer sex.
