Responsibilities Position Description: Case Planner is responsible for clients in New York Foundling’s NY 15/15 & ESSHI Vital Brookdale young adult housing program. Housing young adults age 18-25, who have aged out of care and/or are experiencing chronic homelessness. Case Planner is assigned up to 16 cases. Position reports to Program Supervisor. Conducts home, community, and office visits; provide referrals to community service providers in accordance with the service plan for the participants; accompanies participants to drug treatment, medical, school, housing, human resource administration appointments as necessary. Role requires comprehensive case plan assessments and delivery of appropriate interventions. The Case Planner can expect to spend at least 50% of work time in the field or working remotely and must use tools and policies to record work time and maintain communication and accountability. JOB RESPONSIBILITIES: Case management responsibility for 16 young adults who are living in New York Foundling’s Brooklyn Scattered-Site locations. May be required to report to Foundling’s Manhattan office as necessary. Adhere to Fair Futures Coaching model and provide Core Case Management services for each case, with the goal of supporting participants independent living skills and overall wellness. Coordinate youth’s service needs. Make appropriate referrals and follow-up with participant and service provider to ensure young adult is accessing services. Where youth is not accessing services, coach young adult and advocate on their behalf to help remove barriers. Conduct required monthly in-office and home visits with assigned cases. Complete additional meetings as needed to ensure optimal outcomes for the case. Provide flexible scheduling to complete case meetings and prioritize participant's schedule. Collaborate with program vocational specialist to ensure that they are receiving appropriate vocational and educational referrals and skill building coaching. Work with young adult to facilitate their integration into the community, support their ability to live independently, support the develop of relational skills to secure a broad support network. Supervise or facilitate workshops, field trips, community days and enrichment activities for young adults in the program. Attend trainings and conferences for professional development.
