About The Position

The Case Planner provides casework services to an assigned caseload of children and families in the residential program. This role supports birth parents in meeting court-ordered requirements for family reunification and facilitates concurrent planning to ensure timely permanency for the child if reunification is not possible.

Requirements

  • Bachelor's Degree (experience in child welfare field preferred)
  • Valid New York State driver's license and ongoing automobile insurance
  • Reliable transportation during working hours
  • Strong interpersonal and communication skills; ability to engage effectively with children and adults from diverse socio-economic and cultural backgrounds
  • Physical ability to stand, walk, climb, stoop, kneel, and occasionally lift/move items over 25 pounds
  • Ability to work collaboratively as part of a team

Responsibilities

  • Manage an assigned caseload of children in the Residential program, providing services that ensure safety, well-being, and support toward permanency goals.
  • Provide casework services to birth parents to help them meet court-ordered reunification requirements, while also developing concurrent plans to support timely permanency when needed.
  • Collaborate and maintain ongoing communication with local district case managers and other service providers to advance the child's permanency plan.
  • Support and manage transitions of children between care settings (e.g., foster care, birth parents' home, adoptive home).
  • Participate in Family Court proceedings related to the child's permanency plan.
  • Support foster parents in developing skills to meet the needs of children in their care.
  • Maintain accurate and timely case documentation, progress notes, and required reports in compliance with NYS regulations and district requirements.
  • Model and promote sensitivity, fairness, and acceptance of diversity in all interactions.
  • Participate in program Quality Improvement and Utilization Review processes.
  • Foster positive working relationships within the agency and with external partners.
  • Comply with all agency documentation and vehicle safety usage policies.
  • Promote program services when requested.
  • Participate in staff training, meetings, committees, and agency events.
  • Perform other duties as assigned.
