About The Position

The Case Planner (Solutions-Based Casework) position is a full-time role based in the Brooklyn Prevention Services Center. The role involves working with families to provide essential services, including therapy, advocacy, and case management, while adhering to the organization's mission and values. The Case Planner will maintain a caseload of 12 families, ensuring regular contact and documentation, and will participate in various professional development activities.

Requirements

  • Bachelor's Degree or Bachelor's in Social Work required.
  • Strong interest in learning Family Therapy theories and interventions.
  • Commitment to working in a strengths-based framework with at-risk families.
  • Self-starter with motivation to support families and build client skills.
  • Excellent oral and written communication skills.
  • Ability to manage multiple tasks and respond to crises.
  • Strong engagement skills and experience with children, teens, parents, or families.

Nice To Haves

  • Bilingual in Spanish/English preferred.

Responsibilities

  • Understand and promote the organization's Mission, Vision, and Core Values.
  • Maintain an average caseload of 12 families with a minimum of 2 contacts/month, including home visits.
  • Complete all required documentation for the SBC model and ACS.
  • Provide family and individual therapy, advocacy, educational monitoring, parenting support, and general case management services.
  • Receive training and certification in the Solution Based Casework Model.
  • Collaborate with and refer to appropriate service providers for case coordination.
  • Maintain up-to-date progress notes and complete Family Assessment and Service Plans.
  • Input contact data into PROMIS and CONNX systems.
  • Schedule and participate in Family Team Conferences for all cases.
  • Attend staff meetings and participate in division events and core training.
  • Complete required training hours through the Work Force Institute (ACS).
  • Perform other duties as assigned.
