About The Position

To coordinate implementation of the treatment plan, act as liaison between SAI and the family courts, placing agencies, families and the community. The case planner works to achieve change in the client by influencing the “external” environment such as in residential, school, family, and court settings. The case planner works to support the direction taken by the clinical therapist who effects changes “internally” in the client’s thinking, feelings, and behavioral choices.

B.A. with two years’ experience required

Responsibilities

Provide case planning services to residential and respite youth; provide Aftercare services to youth discharged from the residential program as assigned.
Maintain regular contact with assigned clients/families, placing agencies and outside service providers.
Provide non-clinical and outreach services based on service needs assessment of client caseload, including transportation, home visits, parent skills training, assistance with daily living skills, safety planning, supervised visitation, and additional services as needed.
Initiate and coordinate Individual Crisis Support Plan (ICSP) for each assigned case.
Accompany residents to court appointments and other medical/confidential appointments as necessary.
Provide orientation to residents and families to agency procedures and programs.
Facilitate provision of outreach services (i.e., aftercare, care management, day services, independent living, counseling services).
Complete Family Assessment Service Plans (FASPs) for each client on caseload according to NYS OCFS standards and timeframes
Maintain accountability requirements, including progress notes, Family Assessment Service Plans (FASPs), and contact sheets
Provide family courts and placing agencies with necessary updates and recommendations.
Compile quarterly treatment reports from clinical, educational, and residential staff generated at treatment meeting and send to the placing agency.
Maintain updated progress notes of significant case events, treatment or special meetings and all contacts with the resident, family, and collateral agencies such as LDSS, family courts and other service providers in the NYS OCFS CONNECTIONS electronic case management system.
Send out referral packets and facilitate discharge planning.
Provide the coordination of services and client advocacy to each assigned case
Chair service plan reviews, discharge meetings and special meetings. Attend core, rounds, and staff meetings.
Coordinate home visit arrangements with input from the treatment team.
Arrange family visits and other appointments as necessary.
Follow up AWOCs (Absent Without Consent) making telephone calls to families and placement agencies (and police if necessary) and seeking necessary legal documents (warrants).
Manage and coordinate critical situations and precipitous discharges.
Work cooperatively with resident's therapist, meeting on a regular basis and functioning as a member of the multidisciplinary team
Communicate with Agency and Behavioral Support staff, education and residential staff regarding visitation, court appearances, outside appointments.
Act as liaison with education and work study staff.
Assist other case planners when necessary (i.e., court appointments, transportation).
Act as third-party reviewer for other treatment teams in the absence of the regularly scheduled third party reviewer.
Participate in individual and group supervision.
Attend Clinical Services staff meetings.
Attend all mandatory in-services and outside workshops as required by the agency, plus additional training in evidence-based models and trauma-informed care
Adhere to the NASW, NYS Justice Center and St. Anne Institute Code of Ethics.
Complete other duties as assigned/requested.
