About The Position

In the context of child-centered practice, the Case Planner provides family-focused services to promote permanency, safety, and well-being, while also reducing the length of stay in foster care. The Case Planner delivers comprehensive casework services for both youth and families, ensuring that all case management services comply with ACS mandates. The delivery of casework is consistent with Good Shepherd Services' philosophy and adheres to city and state mandates. This position is based onsite with the possibility of 1 or 2 days remote work. The organization provides a range of programming for vulnerable youth and adolescents with a history of trauma.

Requirements

  • Bachelor's degree required.
  • Experience working with families and adolescents in the child welfare system required.
  • Ability to establish working relationships with participants in need of supportive services.
  • Working knowledge of Connections is required.
  • Excellent verbal and written communication skills are necessary.
  • Ability to work flexible hours, including two evenings.
  • Ability to use public transportation to travel throughout New York City.
  • Commitment to GSS models of practice including Sanctuary and ACS principles to promote permanency, safety, and well-being.
  • Commitment to work from a strength-based, youth development, and trauma-informed perspective.

Responsibilities

  • Maintain primary case planning responsibility for 16-20 cases in collaboration with the social worker.
  • Establish immediate contact with families to provide information about the program.
  • Conduct required casework contacts with the child, parent, and collateral contacts (schools, service providers, medical personnel, mental health practitioners, youth development counselor, and substance abuse counselors).
  • Work collaboratively with Social Worker to make assessment-based referrals as necessary to meet the needs of the child and/or parent.
  • Assess birth parents' homes for diagnostic purposes to ensure the safety of youth and family while at home and complete home safety assessments.
  • Attend court and permanency hearings.
  • Maintain up-to-date case records within agency standards and complete all necessary ACS/Agency documentation including FASPs, Progress notes, Permanency Hearing reports, and all other reports necessary to fulfill the function of the work.
  • Complete and submit progress notes preferably within one day of the event but no later than the end of the given week of the event.
  • Submit PYA, FASP, and Permanency Hearing Report on the internal date for review provided by the agency's compliance department.
  • Manage case load of AWOC and work collaboratively with assigned Social Worker to locate AWOC youth.
  • Develop community relationships and communicate with all collaterals, including ACS.
  • In collaboration with the primary social worker and youth development counselor, work with the full treatment team, family, and youth to develop the most appropriate permanency plan with emphasis on lasting family connections and/or other permanent caring connections in the community.
  • Attend and participate in treatment team meetings, service plan reviews, and FTCs; provide relevant input.
  • Maintain appropriate and relevant documentation of all contacts and events.
  • Effectively use and practice the Sanctuary Model tools such as safety plans, community meetings, psycho-education groups, team meetings, red flag reviews, treatment planning conferences, and self-care in the department/program to create a non-violent and trauma-sensitive culture.
  • Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture.
  • Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss, and Future) Framework into daily practice to ensure quality care and an effective work environment.
  • Provide coverage for Marian Hall cases when necessary.
  • Perform other duties as assigned.
