About The Position

CAMBA’s School-Based Early Support Program is a collaborative initiative with ACS designed to empower families and promote children's well-being within the school environment. This program offers a range of services and supports directly accessible to families through partnerships with local schools. The Case Planner will conduct outreach activities to engage families, develop individualized support plans, and advocate for clients while ensuring child safety and well-being. The role involves managing a caseload, maintaining documentation, and collaborating with various stakeholders to provide comprehensive support to families.

Requirements

  • A Bachelor of Arts (BA), Bachelor of Science (BS), or Bachelor of Social Work (BSW) degree.
  • Minimum of two years of experience working with families and children in a community setting.
  • Strong interpersonal and communication skills.
  • Exceptional writing skills for documentation and reports.
  • Excellent organizational skills and ability to prioritize tasks.
  • Proficiency in Microsoft Office Suite.
  • Commitment to cultural competency and anti-oppressive practice.
  • Strong understanding of student support services, including IEPs and 504 plans.

Nice To Haves

  • Working knowledge of relevant databases and case management systems.
  • Bi-lingual English and Spanish or Haitian Creole.

Responsibilities

  • Conduct outreach activities to engage families and discuss program offerings.
  • Collaborate with referral sources to understand each family's unique strengths and needs.
  • Develop individualized support plans based on comprehensive assessments.
  • Build trusting relationships with clients while maintaining confidentiality.
  • Develop client goals, service plans, and track progress.
  • Act as a client advocate with schools and healthcare providers.
  • Empower clients to overcome challenges and achieve self-sufficiency.
  • Monitor client progress and update plans as needed.
  • Facilitate support groups and events as necessary.
  • Assess children's safety and promote their well-being.
  • Manage a caseload of 12 clients effectively.
  • Maintain accurate documentation and prepare reports.
  • Conduct frequent casework contact with families.
  • Build relationships with families from diverse backgrounds.
  • Educate school staff about the program and target population.
  • Collaborate effectively with program staff and external partners.
