About The Position

The Case Manager at Sunrise Community Health is responsible for delivering patient care services through effective coordination, implementation, and supervision of care for assigned patients. This role emphasizes a quality, customer-first, and compassionate approach to enhance patient health by collaborating with providers, clinical staff, and community resources. The Case Manager will develop client-focused care plans, document patient interactions, and advocate for patients' needs while ensuring access to appropriate healthcare services.

Requirements

  • High School Diploma or GED required.
  • Bachelor's Degree in human services, social work, or nursing preferred.
  • 1-2 years of related experience or equivalent combination of education and experience.
  • Bilingual English/Spanish preferred.

Responsibilities

  • Identify clients that will benefit from care coordination.
  • Work with agencies and service providers to facilitate care coordination and information sharing.
  • Provide case management to assigned patients.
  • Collaborate with providers and clinical staff in a team-based care approach.
  • Review daily schedules and patient charts to ensure efficient visits.
  • Document patient case management plans and ongoing activities in the EHR.
  • Identify patients' insurance coverage and payment sources for services.
  • Address patient risk factors and obstacles to care.
  • Connect patients to services and resources to meet established goals.
  • Educate patients and families about the case management process.
  • Monitor patients according to guidelines set by health authorities.
  • Assist with scheduling appointments, follow-up care, and referrals.
  • Provide health coaching and education for Diabetes patients.
  • Track and distribute insulin samples.
  • Assist patients with transportation needs.
