Gaudenzia's Mission: Gaudenzia's Mission is to promote long-term recovery by providing comprehensive treatment and support to people with substance use disorder, co-occurring mental health disorders, and related conditions, regardless of individual circumstances. This individual is responsible for providing case management services, discharge planning, and care coordination for clients in the program. This person may handle an independent caseload and assist other personnel by ensuring proper referral of clients to all appropriate services and resources available and necessary to them. These services and resources may include medical care, benefits, education and employment, housing, recovery and prevention services, and aftercare supports. Duties may include, but not be limited to providing intake services to clients; preparing reports, discharge/ continuing care plans, and keeping necessary records within client database and electronic health record; coordinating the activities of one or more groups of clients; and generating referrals. Work is performed in accordance with established regulations, and policies and procedures; however, this individual is expected to exercise considerable initiative and independent judgment in achieving objectives.

Requirements

  • Education: Case Managers must have, at minimum, a bachelor's degree in a generally recognized clinical discipline.
  • Case Manager I: Two years' experience providing case management and/ or drug and alcohol counseling
  • Case Manager II: Three years' experience providing case management and/ or drug and alcohol counseling
  • Case Manager III: Three years' experience providing case management and/ or drug and alcohol counseling including at least one year supervisory or administrative experience

Responsibilities

  • Treat all clients and staff with dignity and respect
  • Attend all team / staff meetings as required by Supervisor
  • Role model pro-social behavior
  • May be responsible for conducting and documenting intake interviews, as needed
  • Be responsible for a caseload of clients, as identified by program requirements and in accordance with state regulations
  • Complete progress notes for all client-related activities within expected specific time frames
  • Coordinate with agency and community resources to connect clients to treatment services according to CYS goal plan
  • Establish and maintain positive relationships with service providers and referral sources
  • Assist clients with connecting to resources needed to reach CYS and personal goals during the course of their treatment and in advance of discharge
  • Ensure compliance with all federal, state, local and funding source guidelines
  • Maintain valid US driver’s license and insurance coverage
  • Must have reliable personal transportation
  • Home visits to referred clients for the purpose of Case Management services
  • Analyze UDS as appropriate for caseload
  • Other duties as assigned by Supervisor
