SUMMARY Under general supervision, is responsible for the development and implementation of the Individualized Service Plans for the people we support and their advocates in accordance with the regulations and requirements of the applicable governmental and other regulatory agencies within a residential setting. Provides case management services to assigned individuals and determines individual aptitudes, interests and preferences. Identifies appropriate service providers and links individuals with providers. Coordinates services and monitors the satisfaction of the people we support with the services received.

Requirements

Ability to read, write, speak, and understand English
Ability to speak American Sign Language (ASL) fluently
Good interpersonal skills are required to interact effectively with employees, co- workers, the people we support, their families, and outside contacts.
Excellent organizational skills.
Proficiency in the use of a personal computer. And knowledge of appropriate software
Due to sensitive nature of the position, must be able to maintain the strictest confidentiality.
Ability to work independently and handle multiple tasks.
Ability to work flexible hours.
Bachelor's Degree in the Human Services field
At least one year of prior experience working with people with developmental disabilities

Responsibilities

Provides social services to each person we support working toward maximizing their social functioning, enhancing coping capacity of their family, asserting and safeguarding their human and civil rights, and fostering their human dignity and personal worth.
Develops and monitors individual treatment plan in accordance with regulatory and agency policy.
Participates in interdisciplinary planning.
Interprets, documents, and follows up on all recommendations of interdisciplinary team meetings.
Chairs all meetings of the interdisciplinary team.
Includes channels of communications and consultation with residential staff and professional staff to identify the needs as the people we support progress through habilitation.
Develops goals and activities generated by interdisciplinary team.
Produces an active treatment plan schedule for each person served.
Documents comprehensive functional assessment and review of plan of care as needed.
Ensures proper data collection through observation and in-service training.
Assists in all activities of daily living and participates in activities outside of the residence, i.e. shopping, trips swimming pool as needed.
Supervises Residence Program Specialists in implementation of active treatment plans.
Makes regular and meaningful family, residential, and agency-wide contacts.
Trains and instructs Residence Program Specialists on the specific goal plans for the people we support, relevant data collection, daily routine, and general procedures and practices of the residence.
Maintains contacts with government social services agencies, not-for-profit social service agencies to access resources and/or advocate for services.
Performs individual evaluations to determine whether and when alternate placement is needed.
Handles family outreach and in-service training.
Ensures that all benefits for which the people we support may be eligible for are applied for in an expeditious manner and to effect transfer of any prior accumulated funds to agency.
Provides systematic follow-up to assure referral to appropriate community agencies upon discharge.
Works regularly with, develops, or is responsible for maintaining security of confidential information such as records of person receiving services which requires regular use of discretion. Improper disclosure may cause embarrassment, disruption, or conflict to agency or persons receiving services and their families.
Coordinates admissions to program including preparation of referral material, initial screening of applicants, contact with families, and other referral services.
Ensures all required adaptive equipment is secured and maintained in good working order.
Maintains up to date files on applicants.
Processes new admissions including contact with families and arrangements for day programming.
Advocates for the people we support to ensure that all their rights are being fulfilled.
Writes monthly case notes on all interpersonal, social, workshop progress and family contacts of the people we support.
Serves as a liaison to Day Programs, Service Coordination, and Clinics as needed.
Keeps abreast of changes in field.
Performs other related duties as requested.
