The TLP program provides longer-term, supportive housing and case management for homeless youth ages 16 to 21 who are unstably housed. The program helps participants transition to independence by offering safe, stable housing and comprehensive support services such as life skills training, education and employment assistance, mental and physical health referrals, and family reunification efforts when appropriate. The ultimate goal of TLP is to equip young people with the skills, resources, and stability they need to achieve self-sufficiency and long-term housing stability. This role is hands-on, relationship-focused, and impactful. You will walk alongside youth as they work toward stability — celebrating milestones, building trust, and helping them move into adulthood with confidence.
