We’re looking for gerontologist/ geriatric social worker experienced care manager with at least 1 year experience with seniors and working withing the This role is ideal for a professional who loves client interaction, understands family dynamics, and naturally builds trust. You’ll be the warm, knowledgeable face representing our agency during in-home assessments, care plan creation, and supervisory visits.

Requirements

  • 1+ years Professional Experience
  • Bachelor’s or Master’s in Gerontology, Social Work, Nursing, or related field
  • 1+ years Experience in home care, hospice, case management, hospitals, or senior services
  • Strong understanding of aging, dementia, chronic conditions, and family caregiving dynamics
  • Warm, patient, empathetic communication style
  • Excellent listener who can quickly build trust with families under stress
  • Able to educate, guide, and gently influence decision-making
  • Confident in presenting care recommendations
  • Comfortable enrolling clients in services without pressure
  • Able to read the room and tailor messaging to family needs
  • Approved Live Scan
  • Approved Fingerprinting and Criminal background Check
  • Negative Drug Screen Testing
  • Valid Driver’s License with no Major Violations in Last Five Years
  • Current Auto Insurance Policy
  • Highly organized, reliable, and self-directed
  • Professional presence and excellent boundaries
  • Comfortable with per-diem flexibility and variable weekly hours

Responsibilities

  • Conduct comprehensive asessments: Meets with seniors and families to understand their needs, daily routines, and preferences in order to build a personalized care plan.
  • Develop and implement clear, goal-oriented care plans that address the client's health, care, functional, and quality of life needs.
  • Ensures clients, families, caregivers, and office staff receive the information and support they need to deliver an exceptional care experience.
  • Observes and communicates opportunities to enhance overall client satisfaction and service quality.
  • Performs in-home safety and quality assessments and provides timely follow-up to families, caregivers, and care partners.
  • Coordinates in-home visits, schedules assessments, and works closely with the office team to align caregiver availability.
  • Completes caregiver supervisory visits to ensure care standards are met.
  • Educates families about how changes in a client’s condition may influence required care and support.
  • Help clients enroll into care during or after the assessment
  • Confidently explain services, value, and care recommendations
  • Collaborate with office staff to ensure a seamless start-of-care
  • Submit timely and accurate visit notes, assessments, and care plans
  • Use agency systems to record findings and communicate changes
