About The Position

The Interim Housing Case Manager provides intensive, person-centered case management services to individuals experiencing homelessness who are enrolled in the Pathway Home Program. This role supports participants as they transition from unsheltered homelessness into interim housing and ultimately achieve permanent housing stability. Using a Housing First, trauma-informed, and harm reduction approach, the Case Manager assists participants in addressing barriers related to housing, health, mental health, income, benefits, and daily living needs through individualized service planning and coordinated care. Overall, the Case Manager plays a critical role in helping participants move from encampments into safe housing and onto a sustainable pathway toward long-term stability

Requirements

  • High School Diploma or GED or Equivalent
  • Active driver’s license and up to date car insurance required
  • Experience working with participants with mental illness, chronic health issues, and substance abuse disorders
  • Experience with Outreach and engagement, crisis intervention, suicide assessment/prevention, affordable housing, housing rights etc.
  • Detail oriented with excellent time management skills
  • Strong verbal and written communication skills including knowledge of Microsoft Suite (Word, Excel, Outlook etc), required
  • Ability to self-motivate and be flexible in a fast-paced environment with minimal supervision

Nice To Haves

  • Bachelors degree in Psychology, Social Work, or equivalent
  • Minimum of one (1) year experience in case management preferred
  • Knowledge of HMIS database strongly preferred

Responsibilities

  • Serve a caseload of . clients, providing active case management for all assigned cases, including meeting weekly with each person individually
  • Act as the lead for client’s housing-related case management, maintaining an active housing plan for each client on your caseload, based in creative solutions to homelessness
  • Maintain a current and thorough knowledge of community resources and utilize them to provide comprehensive, wrap-around services to clients, navigating them through the best options for their unique situations
  • Work collaboratively with clients to address barriers to housing, including linkages to income/benefits, mental health services, physical health services, etc.
  • Develop effective, trusting relationships with clients and use a client-centered approach that includes motivational interviewing, harm reduction, and trauma-informed care
  • Advocate for your client’s needs in the shelter and when interfacing with other agencies or service providers
  • Facilitate intakes for individuals who are referred into interim housing
  • Create a supportive environment and ensure clients’ progress, utilizing a strengths-based approach
  • Facilitate wellness, enrichment, and life skills groups
  • Provide crisis intervention when needed
  • Maintain client confidentiality at all times while following agency, state and HIPAA regulations
  • Create and maintain accurate documentation of client information (intake, referrals, progress notes and service delivery) in HMIS database and in accordance with program funder requirements as well as spreadsheets and other reports in accordance with agency requirements
  • Participate in case conferences, individual supervision, and other staff meetings
  • Work in collaboration with other agency and program staff, in order to coordinate services
  • Communicate effectively and in a timely manner with management, peers, and clients
  • Participate in evaluation and trainings in order to better respond to client/community needs
  • Build rapport using a trauma-informed, strengths-based approach
  • Facilitate participant intake and enrollment into interim housing
  • Maintain weekly contact with participants
  • Provide a minimum of two service interactions monthly
  • Staff must transport client in personal vehicle
  • Other duties as assigned
