About The Position

At True North, we are committed to transforming lives impacted by substance use and addiction. Our Intensive Residential Treatment program offers up to 45 days of personalized, harm-reduction, focused care for individuals aged 18 and older. Through structured support, life training, and community reengagement. We help residents build a solid foundation for a healthier, more fulfilling future. Our approach doesn’t stop at treatment; we collaborate with programs like Withdrawal Management, Crisis Response Teams, Intensive Outpatient, Medication-Assisted Treatment, and Recovery Support Services, ensuring comprehensive support for every step of the recovery journey. This position includes eligibility for a monthly performance bonus.

Requirements

  • Have a Bachelors Degree in a Human Services field from an accredited college or university.
  • Have a valid driver’s license and an insurable driving record.
  • Can successfully complete pre-employment screening, including background check, fingerprinting, and drug and TB testing.
  • Enjoy working within legal, justice, and behavioral health systems to support individuals as they work toward competency and stability.

Responsibilities

  • Support Client in Goal Achievement: Assist clients in reaching their goals as outlined in treatment plans or through brief, appropriate interventions.
  • Coordinate and Monitor Care: Collaborate with therapists and relevant staff to address client needs, coordinate services, and prevent deterioration or hospitalization through consistent monitoring within the programs scope.
  • Facilitate access to resources: Refer, coordinate, and assist clients in accessing services such as vocational training, medical care, social security, and housing. Provide aid during application processes and supply supporting documentation as needed.
  • Build Community Partnerships: Maintain cooperative relationships with community organizations, public agencies, and private businesses (e.g., landlords, banks) to deliver supportive services. May manage contracts, leases, and other client related paperwork.
  • Enhance Client Engagement: Facilitate client participation in services and educational opportunities, teach approved psychoeducational curricula, adapt techniques to ensure participant engagement.
  • Promote Daily Living Skills: Educate and guide clients in maintaining daily life activities such as financial management, adherence to medical recommendations, and physical and emotional self-care.
  • Provide Caregiver Support: Offer support, consultation, and education to family members, caregivers, and other stakeholders to reduce barriers and enhance client outcomes.
  • Adapt to Client Needs: Ensure engagement by providing flexible service delivery, including home visits, outreach, accompaniment to appointments, and assistance with medication management when clinically indicated.
  • Develop Client Coping Skills: Help clients improve their well-being by coaching behavioral skills, fostering information decision-making, and developing problem-solving and daily living strategies.
  • Monitor Progress and Maintain Records: Track and document client progress toward treatment goals using electronic systems. Record case management notes, correspondence, and performance data to comply with government requirements.
