About The Position

Case Manager Next Step - Geauga & Lake Counties Full-Time $40,000/Year SCHEDULE: Full-time, determined on a case-by-case basis GENERAL STATEMENT OF DUTIES: Provides community-based case management and advocacy for at-risk youth and young adult (YYA) households facing homelessness.

Requirements

  • Demonstrated organizational, interpersonal, and communication skills
  • Willingness to learn and apply best practice models as appropriate
  • Valid Ohio Driver’s License and ability to meet agency requirements for driving insurability if applicable; reliable transportation
  • Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Identification & Investigation).
  • High school diploma or equivalent required.

Nice To Haves

  • College level coursework in human services or related field preferred.
  • Experience with the issues of homelessness, at risk youth, domestic violence, chemical dependency, and/or mental health preferred.

Responsibilities

  • Outreaches at local programs/agencies to ensure effective local outreach and coordination.
  • Assesses needs, risks, and obstacles applicable to household stability and long-term permanent housing.
  • Assures the provision of financial assistance as needed and requested.
  • Assists participants in developing realistic and achievable housing plans that focus on housing stability and include measurable action steps and timetables.
  • Maintains regular contact with the household to provide crisis intervention, referrals, and ongoing life skills development.
  • Assesses progress toward goals, updating and modifying individual service plans as needed.
  • Documents and records all significant interventions, diagnostic action steps, progress reports, utilization of referral services.
  • Maintains active, organized and professional files on all program participants.
  • Collects and provides required data for the HMIS database.
  • Assures that all applicable participant housing is habitable according to established organizational standards.
  • Actively participates in regular team meetings for the purpose of program consistency, monitoring and effectiveness.
  • Applies best practice models (ie.housing first, trauma informed care) in service delivery approach.
  • Participates in community-wide events promoting services, as applicable.
  • Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
  • Other duties as assigned.
