About The Position

Our Psychiatric Services and Medication Management program supports individuals and families through a coordinated, recovery-oriented approach grounded in research showing that a combination of therapy and medication is often most effective. Services span a full continuum of care—including consultation, prevention, intervention, and aftercare—across outpatient clinics and residential settings throughout North Range. As a Case Manager on the Medical Team, you will play a meaningful role in supporting client recovery and long-term wellness. You’ll work closely with psychiatrists, nurses, and other interdisciplinary team members to ensure services align with each client’s individualized treatment plan. Through collaboration, advocacy, and strong communication, you will help clients and families navigate complex systems of care while building pathways toward stability and independence. In this role, you’ll connect individuals to internal and community-based resources, advocate with federal, state, and local agencies, and help remove barriers to care. You’ll also provide education, skill-building, and ongoing support to promote engagement, resilience, and sustained progress. Requirements

  • Have a Bachelors Degree in a Human Services field from an accredited college or university.
  • Have a valid driver’s license and an insurable driving record.
  • Can successfully complete pre-employment screening, including background check, fingerprinting, and drug and TB testing.

Responsibilities

  • Manage an assigned caseload and provide high-quality, client-centered services under the guidance of administrative and clinical supervisors.
  • Partner with clients to help them achieve goals identified in their individualized treatment plans, supporting progress, stability, and recovery.
  • Collaborate closely with therapists, prescribers, and interdisciplinary team members to coordinate care, address emerging needs, and help prevent crisis, deterioration, or hospitalization through proactive monitoring and follow-up.
  • Assess client needs and make thoughtful recommendations for services, including contributing to treatment planning, delivering appropriate interventions, and supporting transitions between levels of care.
  • Work collaboratively with insurance providers and other North Range Behavioral Health business office sources to ensure services are authorized, appropriately aligned with coverage, and accurately documented for ethical and successful billing.
  • Promote engagement in psychiatric and medical services by providing education, encouragement, and ongoing support to clients and families.
  • Offer consultation, guidance, and education to providers, caregivers, and other involved parties to strengthen coordination and reduce barriers to care.
  • Track, evaluate, and document client progress toward treatments goals using electronic health record systems, including case notes, reports, correspondence, and care coordinator activities.
  • Build and maintain strong working relationships with community partners, public agencies, and local businesses to support client’s stability and access to resources.
  • Assist medical providers with special requests, including care coordination tasks such as pharmacy linkages, documentation support, and letter preparation.
  • Contribute to team success by completing additional duties as needed in support of program operations and client care.
