Our Psychiatric Services and Medication Management program supports individuals and families through a coordinated, recovery-oriented approach grounded in research showing that a combination of therapy and medication is often most effective. Services span a full continuum of care—including consultation, prevention, intervention, and aftercare—across outpatient clinics and residential settings throughout North Range. As a Case Manager on the Medical Team, you will play a meaningful role in supporting client recovery and long-term wellness. You’ll work closely with psychiatrists, nurses, and other interdisciplinary team members to ensure services align with each client’s individualized treatment plan. Through collaboration, advocacy, and strong communication, you will help clients and families navigate complex systems of care while building pathways toward stability and independence. In this role, you’ll connect individuals to internal and community-based resources, advocate with federal, state, and local agencies, and help remove barriers to care. You’ll also provide education, skill-building, and ongoing support to promote engagement, resilience, and sustained progress. This is an opportunity to join a mission-driven team where your work directly contributes to meaningful outcomes for the people and communities we serve. This position includes eligibility for a monthly performance bonus.
