Job Summary: The Case Manager I provides direct support and case management services to individuals and families experiencing domestic violence within the DVSTOP overflow hotel program. This role conducts assessments, delivers crisis intervention, and coordinates services to support client safety and stability using trauma-informed, strengths-based practices. The position works in both office and field-based settings while maintaining confidentiality and connecting clients to appropriate community resources.

Requirements

  • High School Diploma or GED required.
  • Minimum of 1 year of experience working with individuals experiencing domestic violence, homelessness, or other vulnerable populations required.
  • Knowledge of community resources and social service systems required.
  • Experience with trauma-informed care practices required.
  • Must Be At Least 21 Years of Age
  • Class One Fingerprint Clearance Card
  • Central Registry Background Check

  • CPR/First Aid Certification
  • Valid Unrestricted Driver’s License
  • Current Auto Liability Insurance
  • 39 Month Motor Vehicle Record
  • Adhere to all organizational policies, including the Code of Conduct, professional standards, and relevant regulations.
  • As a mandated reporter under Arizona statute A.R.S. § 13-3620, you are required to immediately report any suspected abuse or neglect of a minor to your Supervisor, Manager, Director, Clinical Director, or Chief Officer, who will assist in making the appropriate report to authorities.

Nice To Haves

  • Associate or Bachelor’s Degree in Social Work, Human Services, Psychology, or a related field preferred.
  • Bilingual (Spanish) preferred.
  • Experience providing crisis intervention, case management, or direct client services preferred.

Responsibilities

  • Perform intake screenings and basic needs assessments to ensure referrals align with client needs.
  • Provide case management services to domestic violence clients within the overflow hotel program.
  • Conduct crisis intervention, safety planning, and provide education, emotional support, and referrals to community resources.
  • Provide and coordinate access to daily basic needs and supportive services for clients.
  • Maintain strict confidentiality of all client information, including hotel placement locations.
  • Follow established safety protocols while delivering field-based case management services.
  • Recognize and respond to symptoms of mental health conditions and/or substance use disorders, including awareness of medication side effects and interactions.
  • Establish, maintain, and update accurate client files, case notes, and demographic data in accordance with agency procedures.
  • Document calls, services, and case activity in a timely, accurate, and thorough manner.
  • Coordinate client admissions and discharges in alignment with program policies and client needs.
  • Develop, implement, and monitor strength-based service plans to support client progress and goal attainment.
  • Assist clients in accessing community resources, including scheduling appointments and coordinating services with external agencies.
  • Provide referrals and coordinate transportation to support access to services.
  • Facilitate safe transitions from overflow placements to shelter or stable housing and complete follow-up as required.
  • Communicate and coordinate with hotels, motels, transportation providers, and partner agencies to support client placement and services.
  • Remain available and responsive to client needs during assigned shift hours.
  • Attend required meetings, trainings, and funder-related activities.
  • Maintain cleanliness and organization of workspace, shared areas, donations, and program supplies.
