About The Position

The Case Manager is responsible for working closely with the Therapist(s) to coordinate services for residents within the facility and to assist with the resident’s transition from the facility. This includes coordinating any academic, vocational, social/recreational, and independent living needs within the community for clients within HomeSafe’s residential group homes. Some evenings and weekends are required to successfully fulfill the responsibilities of this position.

Requirements

  • Bachelor’s Degree in Social Work, Psychology, Counseling or related field required
  • Three years relevant experience required
  • Valid Florida’s driver’s license is required.
  • Successful completion of a background check through Clearinghouse (learn more)
  • Schedule includes occasional evening and weekend hours as needed

Responsibilities

  • Coordinate client intake, admissions, and case documentation to ensure complete, accurate electronic records within required timeframes
  • Build and maintain strong working relationships with CBCs, case managers, guardians, legal partners, schools, and treatment team members
  • Complete and track required assessments (including Casey Life Skills Assessments) and support individualized treatment and independent living plans
  • Collaborate with therapists and program leadership to monitor progress, provide updates, and participate in treatment team meetings and staffings
  • Serve as the education liaison, coordinating school enrollment, IEP meetings, academic monitoring, and communication with school personnel
  • Implement and facilitate independent living and life skills programming, including group sessions, curriculum delivery, and community-based learning opportunities
  • Support discharge planning and aftercare by coordinating placements, resources, follow-ups, and independent living team services for transitioning youth
  • Maintain compliance through timely documentation, outcome tracking, staff training support, and ongoing coordination across systems of care
