Legacy Behavioral Health Services is seeking a case manager to join our team in Lowndes County. Under general supervision performs a wide range of tasks in the community to assist clients in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to clients and their families. Monitor client progress towards treatment goals. Extensive documentation required; accurately charts services and client progress. Must be able to work well with community agencies and function as part of a team.. The Case Manager will be supervised by the Clinical Team Leader.

Requirements

  • Completion of a bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field AND one year of full-time experience in social services delivery.
  • Knowledge of mental illness
  • Valid GA driver’s license with less than 5 points
  • Must be physically and emotionally fit to work in a stress-filled environment to meet deadlines and deal with routine and crisis situations.
  • May have extended periods of sitting, walking, and standing.
  • Requires varying hours of duty to provide monitoring, educational and/or crisis handling, as well as travel to off campus locations in BHSGA or personal vehicle.
  • Must be able to lift fifteen to twenty-five pounds and endure extended periods of standing on a daily basis.
  • While performing the duties of this job, the employee is regularly required to talk and hear.
  • This position is very active and requires standing, walking, bending, kneeling, stooping and crouching as well as lifting and supporting the individuals served.
