About The Position

ServiceNet is looking for a compassionate and forward-thinking Case Manager to join our team at Our Friends Shelter . In this role, you will help individuals and families experiencing homelessness build the foundation for long-term stability. Through direct support, advocacy, and collaboration, you will assist guests in accessing essential resources as they work toward permanent housing. At ServiceNet, you will make a real difference. Requirements

  • High school diploma or equivalent required, with additional training equivalent to two years of college
  • At least one year of experience working with individuals affected by poverty, mental health challenges, and/or addiction
  • Familiarity with local social service agencies and available community resources
  • Valid driver's license and an acceptable motor vehicle record

Responsibilities

  • Conduct comprehensive intakes and assessments for all new shelter guests
  • Support and advocate for individuals and families in shelter or transitional housing
  • Help participants access and follow through with treatment, medical, vocational, housing, and community-based resources
  • Hold weekly meetings with guests to review service/treatment plans, set goals, make referrals, and monitor progress
  • Foster a positive and supportive residential environment in collaboration with participants and shelter staff
  • Maintain clear, accurate case management and progress notes, as well as other assigned documentation
