About The Position

We are looking for energetic, compassionate and empathetic people who want to make a difference in the lives of people experiencing homelessness in Philadelphia. FaSST/Connections CTI Resource Coordinators engage individuals experiencing homelessness with a history or present struggle with mental health challenges and/or substance use. The Case Manager is responsible for identifying and coordinating resources to meet the individualized needs of program participants. This role ensures implementation of service and treatment plans, advocates for participants to access necessary services, and helps identify program needs and service gaps while recommending improvement strategies. The Case Manager works collaboratively with program participants, clinical teams, and community partners to support recovery, stability, and overall wellness. All responsibilities are performed in alignment with APIS and RHD organizational values.

Requirements

  • Bachelor’s degree in Human Services or a related field preferred, or equivalent experience.
  • Intermediate computer proficiency.
  • One year of case management experience preferred.
  • Valid Driver's License held for a minimum of three years
  • Minimum two years of experience supporting individuals in: Behavioral health Addiction recovery Homeless services Behavioral Health Home (BHH) or IDD services.

Responsibilities

  • Participate as a member of a multidisciplinary team to provide holistic, recovery-oriented services.
  • Assist in developing and reviewing recovery plans in collaboration with program participants, clinical staff, medical providers, and other stakeholders.
  • Build and maintain relationships with community organizations and service providers to connect participants with available resources.
  • Provide and coordinate community-based services that support recovery plan goals.
  • Assist participants with accessing services such as housing resources, healthcare, employment, education, and community supports.
  • Support participants in building independence, community connections, and engagement in services.
  • Travel within the program’s service delivery area and transport participants to appointments when needed.
  • Complete required assessments, documentation, and service records according to agency, funder, licensing, and accreditation standards.
  • Maintain accurate and timely case documentation in electronic systems.
  • Participate in team meetings and case reviews.
  • Perform other duties as assigned.
