About The Position

First Episode Psychosis (FEP) refers to the initial occurrence of symptoms characteristic of psychotic disorders, such as schizophrenia or schizoaffective disorder. The First Episode Psychosis (FEP) Case Manager will provide comprehensive case management and support for individuals experiencing their first episode of psychosis. This role involves working within a multidisciplinary team to deliver person-centered care, coordinate treatment, and connect clients with community resources to support their recovery and well-being. The Case Manager will play a crucial role in early intervention, crisis management, and family engagement.

Requirements

Bachelor’s degree in social work, psychology, psychosocial rehabilitation or related human services field -OR- Associate’s degree in related field and two years’ experience working with individuals with mental illness
Minimum of 2 years of experience in mental health case management or related roles.
Strong assessment and diagnostic skills.
Excellent crisis intervention and de-escalation techniques.
Effective communication and interpersonal skills.
Ability to work collaboratively within a multidisciplinary team.
Knowledge of community resources and support services.
Familiarity with evidence-based practices for psychosis treatment.
Valid driver’s license and reliable transportation.
Successful completion of ANSA.
Empathetic, culturally competent, and dedicated to providing high-quality, person-centered care.
Ability to perform job duties in a variety of settings, including community-based locations.
Ability to perform activities such as standing, walking, and sitting for extended periods.
Ability to type for extended periods of time to enter notes.

Nice To Haves

Experience with early psychosis intervention is highly desirable.

Responsibilities

Client Assessment and Care Planning: Conduct thorough assessments of clients presenting with first episode psychosis to determine their needs, strengths, and treatment goals.
Develop and implement individualized care plans in collaboration with clients, their families, and the multidisciplinary team.
Monitor and evaluate client progress and adjust care plans as needed to ensure optimal outcomes.
Crisis Intervention: Provide crisis intervention and de-escalation as needed, ensuring the safety and well-being of clients.
Coordinate with emergency services and other providers to address urgent needs and ensure appropriate follow-up.
Treatment Coordination: Facilitate coordination among various healthcare providers, including primary care physicians, psychiatrists, psychologists, and social workers outside of the agency.
Ensure seamless integration of services and communication between the client, their family, and the treatment team.
Demonstrate basic knowledge of psychiatric medications and their effects.
Is comfortable administering psychiatric medications to support clients in their medication treatment goals. (8-hour training will be provided by MFS.)
Resource Connection: Identify and connect clients with community resources, including housing, vocational training, and social services.
Provide guidance and support in accessing necessary services and navigating the mental health system, including needs related to health insurance benefits and social security benefits.
Family Engagement: Work closely with families to provide education about psychosis, treatment options, and how to support their loved one.
Offer guidance and support to help families manage the impact of psychosis on their family dynamics.
Documentation and Compliance: Maintain accurate and timely documentation of client interactions, assessments, treatment plans, and progress notes.
Ensure compliance with legal, ethical, and organizational standards in all aspects of case management.
Advocacy and Support: Advocate for clients’ needs and rights within the mental health system and broader community.
Promote recovery-oriented practices and work to reduce stigma associated with psychosis.
Collaboration and Teamwork: Actively participate in weekly team meetings, case reviews, and other collaborative activities.
Contribute to a positive and supportive team environment and foster frequent and effective communication with all team members.
Professional Development: Stay current with best practices and evidence-based approaches in the treatment of psychosis.
Pursue ongoing professional development and training opportunities to enhance skills and knowledge.
