About The Position
Case Manager - Solutions Program (KRCC- Lee County) Title: Case Manager Location: Kentucky River Community Care. Lee County Credentials: BA/BS plus one (1) year relevant work experience. Employment Type: Full-Time Skills: Organization, maintain case load, time management. Primary Functions: To assess for strengths and basic needs of the individual requiring case management services. To create a plan with the clients and treatment team for what services the client needs and goals they would like to achieve. Help the individual to access needed resources and services. Identify and assist in finding support systems for the individual to succeed in recovery. Monitor progress the individual is making towards achieving goals. Shifts/Day and Work Hours: Monday through Friday; 8 am- 4:30 pm. Funded in part or in whole from Federal, CHFS, and/or other state funds.
