About The Position

The Case Manager provides case management, care coordination, support and other various clinical services for clients receiving outpatient behavioral health or social services. This role assesses client needs, develops individualized service plans, and connects clients with appropriate community resources under the direction of the Clinical Supervisor, as needed.

Requirements

  • Associate’s degree in behavioral sciences or related field, required
  • 2 years’ experience in direct clinical treatment services
  • 1 year experience with the treatment of adults, children and their families
  • Knowledge and skills to work with chronically mentally ill population
  • Knowledge of community resources
  • Knowledge and experience working with older adults
  • Strong family and individual clinical case management skills
  • Possess and utilize effective verbal and written communication skills
  • Ability to be self-motivated, work independently and as a team member
  • Ability to represent the agency in a professional manner within the community
  • Culturally competent in delivering services to individuals from distinct backgrounds
  • Requires work in clients’ homes and other community location
  • Requires the ability to tolerate exposure to tobacco smoke, domestic animals, household pets and other conditions common to a variety of home environments
  • Agency Affiliated Counselor Registration or Certification
  • Valid Washington state driver’s license and minimum required liability insurance for WA State
  • Must be deemed insurable as determined by Catholic Charities’ insurance provider
  • Being cleared by criminal background check and fingerprinting when required

Nice To Haves

  • Bachelor’s degree in behavioral sciences or related field, preferred
  • Bilingual (English/Spanish) preferred, but not required

Responsibilities

  • Provides case management and therapeutic services, including education and referrals to individuals who meet access to care criteria
  • Collaborates with clients to develop and carry out individualized treatment plans, focusing on client strengths, goals and addressing any risk factors
  • Completes progress notes and other required documentation within the required timeframes
  • Assisting with applying for benefits
  • Developing daily living skills and psychosocial support
  • Service coordination with other providers, including health care professionals
  • Assisting with transportation to and from various appointments
  • Accessing community resources
  • Meetings with individual and/or family member
  • Obtaining housing or assisting with housing retention
  • Establishes and maintains working relationships with community resources
  • Participates in supervision, staff meetings, multi-disciplinary team meetings and staff cases, as required
  • Represents the organization professionally in all interactions
  • Follows agency policies regarding confidentiality in accordance with HIPAA regulations and other consumer rights
  • Participates in ongoing Continuous Quality Improvement to ensure the services provided demonstrate best practice standards and meet all contractual requirements
  • Perform other duties as assigned
