OVERVIEW The Preferred Communities program supports the successful resettlement and integration of especially vulnerable groups of recently arrived refugees. The Case Manager will assist targeted clients with gaining access to needed medical, social, educational, and other services, while also serving to enhance the capacity of the Field Office and the local community to serve and support this population. The Case Manager will provide intensive case management support, access to English language assessment and training, referral to appropriate community support services, and transportation support. The position will report to the Field Office Director in Albany, NY.

Requirements

  • Bachelor's degree in social work, counseling, public health, psychology, or a relevant field preferred
  • Experience in coordinating services, volunteer recruitment and capacity development initiatives required
  • Excellent interpersonal skills, including ability to listen, communicate clearly, and build relationships and partnerships with program and Field Office staff
  • Highly adaptable and flexible with the ability to multi-task and prioritize in a demanding work environment
  • Must have a valid state-issued driver's license, automobile insurance, clean driving record and reliable personal transportation
  • Ability to travel locally as required
  • Covid-19 vaccination card mandatory
  • Proficient in Microsoft Office, specifically Word, PowerPoint, and Excel and other database programs as required

Responsibilities

  • Provide culturally and linguistically appropriate intensive case management services, including assessing clients' needs, assisting clients in developing Self-Sufficiency Plans, achieving stability in areas of vulnerability, and developing the ability to live independently of ongoing agency support;
  • Make referrals to support services and programs that can improve clients' stability and quality of life;
  • Work to connect clients with supportive community networks and support systems to ensure their long-term integration in the community;
  • Orient refugee clients to community resources and provide cross-cultural information;
  • Ensure that refugees are being served in a linguistically and culturally appropriate manner in internal and external programs, providing education and advocacy when necessary;
  • Maintain case files and submit written reports as required;
  • Perform other job-related duties as assigned by supervisor.
