Founded by emergency room psychiatrists, their physician-led, data-driven model has over 15 years of crisis care expertise and is recognized as a national best practice. Their mission is to provide immediate care to people in crisis and connect them to long-term community support. The RESET Center, operated by Connections in San Francisco's South of Market neighborhood, offers an alternative destination for individuals publicly intoxicated due to alcohol and/or drugs, diverting them from emergency departments or jail. This collaboration with local law enforcement and public health agencies aims to improve outcomes, reduce systemic burden, and connect individuals to needed community resources. The Case Manager at the RESET Center provides person-centered care coordination, engagement, and discharge planning for individuals in the sobering program. This role operates from a harm-reduction and trauma-informed approach, supporting participants in safe stabilization, exploring immediate needs, and connecting them to ongoing community services. The CM conducts brief screenings and assessments, engages in crisis de-escalation, and collaborates with community partners to facilitate rapid discharge and seamless linkage to care, ensuring participants have clear next steps and access to resources for health, recovery, and stability.

Requirements

  • 1-3 years of experience in harm reduction, crisis services, behavioral health, or related community care settings
  • High School diploma or equivalent
  • Experience supporting law enforcement diversion or working alongside first responders, emergency departments, or criminal-legal partners
  • Must obtain Overdose Prevention and Narcan Administration training within 30 days of hire
  • Basic Life Support (BLS) Certification
  • Must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment

Nice To Haves

  • Prior experience with Managed Alcohol Programs (MAP), medical respite, interim housing, or sobering programs
  • Associate Degree in Behavioral Health or related field
  • Lived experience related to substance use, recovery, homelessness, or justice system involvement is highly valued and encouraged

Responsibilities

  • Welcome individuals with empathy, respect, and a non-judgmental approach in a low-barrier, trauma-informed environment.
  • Engage participants using motivational interviewing to identify needs, goals, and readiness for services.
  • Conduct brief screenings and assessments to understand risk, safety, and resource needs.
  • Provide education, encouragement, and practical support tailored to participant goals.
  • Develop short-term, accessible discharge plans for individuals staying between 4–24 hours.
  • Coordinate referrals and warm handoffs to community resources including: Substance use treatment (outpatient, detox, MAT), Managed alcohol programs, Mental health providers, Shelters, crisis housing, and social services, Hospital or emergency care when clinically appropriate.
  • Support crisis de-escalation and conflict intervention in collaboration with the care team.
  • Document timely and accurate services entries in Epic to support care coordination, safety, and communication.
  • Maintain collaborative relationships with law enforcement, public health partners, and community agencies.
  • Complete required RESET Center trainings.
  • Perform all other duties as assigned.
