About The Position

Community Health Center of the North Country seeks a compassionate and resourceful Case Manager to join our Federally Qualified Health Center’s integrated care team. In this role, you’ll work directly with patients to identify needs, reduce barriers, and connect them with essential medical, social, housing, and community resources. Position Summary: The Case Manager will have knowledge of community services and resources to facilitate referrals for services for patients who present with needs or barriers to receiving care. This will include establishing eligibility for and gaining access to Federal, State, and local programs that provide or financially support the provision of medical, social, housing, educational, or other related services. The Case Manager works closely with a diverse population, assisting with their needs while remaining culturally competent.

Requirements

  • An associate degree in a health or human services field is preferred; some related experience required.
  • Ability to work independently with minimal supervision. Meet with the supervisor on a regular basis to discuss emerging issues and progress.
  • Ability to establish and maintain effective working relationships with staff, patients, family members, the public, and representatives of other agencies and organizations.
  • Ability to effectively communicate and work cooperatively in an interdisciplinary team setting.
  • Valid New York State Driver’s License with an acceptable motor vehicle abstract.
  • Reliable personal vehicle for business use
  • Ability to lift and carry up to 25 lbs.
  • Excellent hand-eye coordination.
  • Ability to sit for long periods of time.
  • Ability to tolerate extensive exposure to a computer monitor/screen.
  • Ability to tolerate extensive use of computer keyboards

  • Knowledge of available community resources and their organization, purpose, and use is preferred.

Responsibilities

  • Assist patients applying for the Sliding Fee Scale discount program and process the applications with assistance from the Finance Department.
  • Assist patients in navigating billing issues with local hospitals and other provider offices.
  • Assist patients with transportation barriers while working with various transportation service providers.
  • Facilitate translation services for patients as needed for visits within the Health Center.
  • Facilitate referrals to external agencies for patients needing external services.
  • Responsible for upholding the caseload of patients and completing monthly interactions & documenting appropriately.
  • Responsible for completing phone calls to patients who have been discharged from the Emergency Department to schedule follow-up appointments
  • Manage medical equipment/supplies loaner program for Health Center.
  • Process prior authorizations for durable medical equipment
  • Follow up with patients who no-show appointments to determine barriers to attending appointments.
  • Facilitate Prescription Assistance Program and DSS Personal Care Forms for patients.
  • Participate in outreach activities and educational sessions focusing on health risk factors.
  • Serve as a liaison between the clinic and other treatment providers, community groups, and social service agencies.
  • In conjunction with the Marketing and Communications and Health Center staff to market services to the uninsured/underinsured individuals through health fairs, screenings, newspapers, radio, and TV ads.
  • Attend and participate in meetings to increase the knowledge base and provide valuable input to help improve agency services and/or as requested by the supervisor.
  • Assist Outreach/Enrollment Coordinator as necessary.
  • Perform other duties as requested by the supervisor.
