About The Position

We are seeking a bilingual (Spanish/English) Case Manager to provide case management to clients with multiple medical and psychosocial needs and across the HIV continuum of care. The Case Manager will coordinate psychosocial support services with community-based social services agencies and on-site substance abuse case managers. They will complete all paperwork, act as an advocate for clients with community-based organizations and public services to reduce barriers and enhance access to care. Will also act as liaison with medical providers and Moore Clinic Social Workers as needed. The Case Manager will participate in relevant training, clinical supervision, conferences, and meetings, and maintain operative knowledge of existing resources for individuals with HIV/AIDS.

Requirements

  • Bachelor's Degree in Social Work, Psychology, or related field.
  • One year of related human service experience, preferably in case management.
  • Additional education may substitute for required experience, and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula.

Nice To Haves

  • Three years of case management experience in a healthcare or public welfare setting with knowledge of HIV, substance abuse, and psychiatric illness preferred.

Responsibilities

  • Perform psychosocial intake assessments and develop care plans.
  • Assist patients in improving their adherence to HIV care and other medical illnesses.
  • Assess and refer patients for mental health, substance abuse programs.
  • Assist patients in applying for health insurance and prescription programs such as ACA, MADAP, Medicare, Medical Assistance, Social Security Extra Help, QMB, SPDAP.
  • Refer patients to the Department of Social Services for programs such as Food Stamps, TDAP, TCA, Eviction Prevention, and Energy Assistance.
  • Provide education on the disability programs provided by the Social Security Administration, such as SSI and SSDI.
  • Refer patients to community resources programs for housing/shelters, food services, and employment.
  • Submit Ryan White voucher requests for rent/utility assistance, and DME.
  • Assess patients for medication purchase and co-payment vouchers.
  • Arrange patient's transportation services that include cabs, tokens, and referrals to the Moveable Feast transportation program.
  • Assist patients in enrolling in transportation programs offered by the MTA and Medical Assistance.
  • Perform other duties as assigned or required.
