About The Position

Case Manager – Skilled Nursing Facility (WA Office) Make a meaningful impact every day. We are seeking a detail‑oriented, clinically knowledgeable Case Manager to support our skilled nursing facility by ensuring accurate authorizations, timely reviews, and strong communication between residents, payors, and clinical teams. If you excel at navigating insurance processes and advocating for patient needs, this role is an excellent fit.

Requirements

  • Healthcare experience required (skilled nursing preferred)
  • Strong understanding of insurance processes, authorizations, and medical necessity criteria
  • Excellent communication, organization, and follow‑through
  • Ability to interpret clinical documentation and collaborate effectively with clinical teams
  • Detail‑driven, proactive, and comfortable in a fast‑paced environment

Nice To Haves

  • LPN or RN license preferred, but not required

Responsibilities

  • Review and submit insurance authorizations for skilled nursing services
  • Monitor authorization status, follow up with payors, and ensure timely approvals
  • Review and respond to denials, including gathering documentation and coordinating appeals
  • Collaborate with nursing, therapy, and medical teams to ensure accurate clinical documentation
  • Maintain organized, compliant records for all authorizations and payor communications
  • Communicate with residents, families, and interdisciplinary teams regarding coverage and care plans
  • Support discharge planning and transitions of care as needed
