General Description: The Case Manager is responsible for providing direct services to tenants including assessment, goal setting and evaluation through formal and informal settings. Essential Duties and Responsibilities: As an onsite case manager in a permanent supportive housing program, you are responsible for assisting and supporting tenants in maintaining stable housing. This entails a variety of duties, including: Meeting all residents on assigned case load to ensure monthly goal plans and related documents are completed as prescribed. Providing referrals and/or support in maintaining connection to community health services, mental health services, vocational programs, chemical dependence treatment, etc. Working with the residents and property management to ensure that the resident fulfills obligations as a tenant. Through the combined use of personal vehicles, public transportation and agency vehicles, case managers will actively assist clients with conducting activities in the community. Crisis intervention and support. Supporting families to ensure their children’s physical, emotional, and educational needs are met and facilitating the children’s participation in developmentally appropriate activities both at Seneca Heights and in the community Leading groups as assigned. Participating in social activities including celebrations and other on-site events. Working collaboratively as a team member to utilize donations, outside resources, and on-site services to meet residential needs. Thorough record-keeping, gathering of information for required reports, and updating of HMIS database as well as other administrative duties as requested. Other duties, as necessary.
