The Holocaust Survivor Support Services Case Manager provides compassionate and comprehensive case management to Holocaust Survivors in Sarasota and Manatee Counties. This position works to connect eligible clients with financial assistance, home care, community-based services, and other essential resources. The role involves direct client contact, care coordination, crisis intervention, and advocacy, ensuring that each client receives personalized and trauma-informed support. This is a part-time position averaging about 20 hours per week, with a case load of about 30 clients.

  • Conduct timely screening and assessment of Holocaust Survivors to determine eligibility for services.
  • Develop individualized care plans and coordinate access to financial, medical, and supportive services.
  • Recommend appropriate use of program resources to maximize impact and client well-being.
  • Provide referrals and ongoing assistance related to community supports, including services through GCJFCS, the Claims Conference, and the German government.
  • Maintain accurate, up-to-date case records in accordance with JFCS standards and professional documentation protocols.
  • Build trusting relationships with clients and families, providing support in a culturally sensitive and client-centered manner.
  • Deliver crisis intervention services to prevent or resolve urgent challenges and promote client safety and stability.
  • Collaborate with the Jewish Federation’s Holocaust Education staff to support program initiatives and community events.
  • Respond to new intakes within 24 hours or the next business day.
  • Participate in the development of outcome measures, track program statistics, and submit timely reports as required.
  • Comply with all JFCS policies, including HIPAA and protocols for the protection of Protected Health Information (PHI).
  • Adjust work schedule as needed to accommodate client availability for office and in-home visits.
  • Support program outreach and marketing efforts within the Jewish community.
  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field required.
  • Minimum two (2) years of experience in social services, preferably with case management responsibilities.
  • Experience working with older adults or trauma-affected populations strongly preferred.
  • Demonstrated ability to establish rapport and build trust with clients and families.
  • Strong understanding of trauma-informed care principles.
  • Knowledge of community resources and systems of care for older adults.
  • Excellent time management and organizational skills.
  • Ability to prioritize tasks and manage multiple responsibilities effectively.
  • Skilled in de-escalating clients experiencing emotional distress.
  • Strong written and verbal communication skills.
  • Computer proficiency, including ability to maintain electronic case notes.
  • Valid driver’s license and ability to travel throughout the community for home visits.
  • Completion of all JFCS mandatory trainings.
  • Participation in workshops and continuing education related to case management, trauma-informed care, Holocaust survivor needs, and related topics.
  • Desk work with computer usage for extended periods.
  • Must have reliable transportation and be able to travel throughout Sarasota and Manatee Counties to conduct home visits and attend outreach events.
  • Russian/Ukrainian language a plus.
